WELCOME!
Poly looks forward to beginning the 2010-2011 school year with both new and returning students.  

Your child's teachers are eager to provide opportunities for critical thinking and interactive learning—both in the classroom and during co-curricular activities in the studio, on the playing field, and in after-school clubs.  Poly also believes that our highly-trained, committed faculty can stimulate and transform gifted students into a powerful community of learners.

As part of Poly's continuing greening commitment, we have created a web-based, Start-of-School information delivery system.  PLEASE READ AND FOLLOW ALL 4 STEPS OUTLINED BELOW ON THIS PAGE.

Poly's "Start-of-School" pages will help you and your child get ready for school next fall.  We also hope that these new web pages will provide increased ease of use for parents. 

As additional 2010–2011 Start-of-School information becomes available, we will update parents.
STEP 1: Infosnap Submission
New & Continuing Parents:

You should have completed your Infosnap submission by Monday, May 31, 2010. NOTE: The Infosnap system is now closed.
 

In addition, however, once you click on the Infosnap link, you will need to enter your child's individual "snapcode."  This password was provided in an email from Poly's Admissions Office on Wednesday, May 12.

If you cannot locate your child's snapcode, please contact Mr. Charles Polizano, Network Administrator, here.
STEP 2: 2010-2011 Health Forms
Poly requires completion of all health forms in order for your child to begin school.  Please avoid unnecessary stress on your child by completing and submitting these forms as soon as possible.  Children may feel singled out if they are unable to participate in school activities because their health forms are not on file.  

Poly's required health and emergency forms are available below.


Middle and Upper School Health Forms

The following forms must be completed for ALL new and returning Middle and Upper School students:

2010 US/MS Participation and Medical Authorization Form
2010 US/MS Student Health Form

2010 US/MS Student Medication Form
2010 US/MS Allergy Action Plan Form

Lower School Health Forms

The following must be completed for ALL new and returning Lower School students:

2010 LS Student Health Form
2010 LS Student Medication Form
2010 LS Allergy Action Plan Form
STEP 3: PERMISSION-T0-REGISTER PROCESS
POLY PARENTS:

The Business Office will mail Permission-To-Register letters to you during the last week of August.

Poly must place a hold on your child's Permission-To-Register if tuition payments are not up-to-date.  Students without Permission-To-Register may not collect their textbooks or attend classes.

For your convenience, you may pay Poly tuition online via credit card here.
