Applying to Middle School

Please review the application checklist and important dates for the 2027-2028 school year.

A completed Poly application includes multiple required items. The checklist below will help you track each requirement.

Middle School does not have strict age cutoffs, though most students follow the September 1 guideline. This means most students entering Grade 5 are 10 years old, Grade 6 are 11 years old, and so on.

You must have a Ravenna account to access the application portal. Once your account is active, you will be able to view application details and register for events.

Middle School Checklist
  • GRADES 5-6

    Grades 5 and 6 are the main entry points for Poly Prep’s Middle School. We typically enroll 15-18 new students in Grade 5 and about 35-38 new students in Grade 6.

    What to Expect

    We look forward to connecting with your family. Our process is a holistic one designed to get to know students in a thoughtful, age-appropriate setting. It begins with an optional open house, continues with the student and family in-person interview, and concludes with a comprehensive review of all of your child’s materials, including academic records, teacher recommendations, writing, standardized testing, and other submitted materials.

    How to Apply

    1. Submit the Preliminary Application: Deadline: November 20, 2026
      Complete the online application in Ravenna, including a short questionnaire, student photo, and upload of your child’s 2025-2026 final report card.
    2. Student and Parent/Guardian Interview
      Register in Ravenna for a Grade 5/6 Student and Parent/Guardian In-Person Interview. Students participate in a small group interview with a faculty member during one of our after-school sessions, which also includes a short writing activity. While students are interviewing, parents/guardians meet with a member of the Admissions Committee for a conversation.
    3. Financial Aid Application (Optional): Deadline: December 15, 2026
      Families who wish to apply for financial aid should complete an application through Clarity.
    4.  Supporting Materials: Deadline: January 8, 2027
      ISAAGNY forms are shared across participating schools.

      • Parent/Guardian Short Answer Questions (ISAAGNY Parent Statement)
      • ISAAGNY Student Interests and Activities Form
      • ISAAGNY Student Common Essay
      • SSAT or ISEE (no testing preference; we superscore multiple submissions)
        • Scores must be received by the application deadline
        • We recommend taking the test by December 31 to ensure timely results
      • Math and English teacher recommendations
        • Please request both of these forms from your child’s current teacher after November 1 to allow teachers time to get to know your child. We will not accept recommendations that are submitted before November 1.
      • Academic Records
        • Official academic record from 2024-2025 and 2025-2026 submitted directly to us by your child’s school using the transcript request form
        • Fall report card from the current academic year


    Decision Timeline
    Poly Prep follows ISAAGNY guidelines. Admissions decisions for Grades 1-8 will be released on February 10, 2027, and families must respond by February 26, 2027.

  • GRADES 7-8

    Grades 7 and 8 are considered non-entry years at Poly Middle, although a small number of students may join in these grades. The Admissions Committee reviews applications for interviews on a rolling basis up to the application deadline. Submitting an application does not guarantee an interview. Selections are based on the preliminary application materials submitted and space availability. 

    What to Expect

    We look forward to connecting with your family. We carefully review preliminary materials, and selected students and their families engage in a one-on-one interview process with a member of the Admissions Committee. All completed applications receive a holistic review, including academic records, teacher recommendations, writing, standardized testing, and other submitted materials.

    How to Apply

    1. Submit the Preliminary Application: Deadline: November 6, 2026
      Complete the online application in Ravenna. The Preliminary Application includes the following:

      • Application questionnaire
      • Student photo
      • 2025-2026 report card
      • Changing Schools Essay
      • Parent/Guardian Short Answer Questions
      • Student Interests and Activities Form
      • Student Common Essay
    2. Student Interview and Parent/Guardian Conversation (by invitation)
      If selected, families will receive an emailed code with which to self-register for a Grade 7/8 Student and Parent/Guardian In-Person Interview. Students and at least one parent/guardian will participate in a one-on-one, in-person interview with a member of the Admissions Committee during the school day. The visit includes a 30-minute writing assessment and a student-led tour of campus.
    3. Financial Aid Application (Optional): Deadline: December 15, 2026
      Families who wish to apply for financial aid should complete an application through Clarity
